Leaving the hospital is not the end of the danger. Clinicians and researchers refer to the weeks that follow as post‑hospital syndrome: a stretch of acquired, transient vulnerability when patients are weakened by the hospitalization itself, sleep‑deprived, deconditioned, and juggling new medications. It is when people are most likely to land back in a hospital bed, and nearly one in five Medicare patients does within thirty days, often for a problem different from the one that put them there the first time.

The frustrating part is that we know what helps — because our founding team has spent decades on exactly this problem: naming post‑hospital syndrome, testing what protects patients through it, and showing that one thing makes the difference: a multidisciplinary care team that follows the patient home, catches trouble early, solves problems and overcomes challenges, and supports recovery day by day. Giving every patient a team like that has never been sustainable. Until now.
